Geisinger Fall 2026 Physical Exam Instructions
NOTE: This form will be posted once it becomes available. This is the information for the Geisinger physical exam for 2026 fall sports.
PIAA CIPPE Section 8 – Physician’s Re-Certification Form
This form must be completed by a physician for any student who answered “Yes” to any of the six SUPPLEMENTAL HEALTH HISTORY questions on the Section 7, “Re-Certification by Parent/Guardian” form.
PIAA CIPPE Section 6 – Physician’s Certification
This is the form that must be completed and signed by a physician in order to participate in athletics.
